Kemoterapi Alan Kanser Hastalarında Akupresurun Konstipasyon ve Yaşam Kalitesi Üzerine Etkisi: Sistematik Derleme ve Meta-Analiz

Abstract

Bu sistematik derleme ve meta- analizin amacı, kemoterapi gören kanser hastalarında konstipasyonu önlemek için akupresur tedavisine ilişkin randomize kontrollü çalışmalardan (RKÇ) elde edilen mevcut kanıtları değerlendirmektir. Kapsamlı literatür taraması 1 Kasım 2023- 1 Nisan 2024 tarihleri arasında Pubmed, Medline, Scopus, Science direct, Web of Science, Cochrane Central Register of Controlled Trials, Dergipark ve YÖK TEZ veri tabanlarında yapılmıştır. Bu meta-analize 8 RKÇ dahil edilmiştir. Araştırmanın normal örneklem hacmi 640 (Deney: 320 Kontrol:320)' dır. Çalışmaların yanlılık riski, "Revize Edilmiş Cochrane Randomize Çalışmalar İçin Yanlılık Riski Aracı" (Revised Cochrane risk-of-bias tool for randomized trials) (ROB 2) kullanılmıştır. Bu çalışmanın meta analizlerinde Rewiev Manager 5.4.1 programı kullanılmıştır. Bu meta analizde uygulanan basamaklar PRISMA 2020 kontrol listesine göre raporlanmıştır. Çalışmanın birleştirilmiş sonuçları girişim sonrasında hastaların konstipasyon düzeyinde 3.29 puan azalma olduğu ve bu azalmanın istatistiksel olarak anlamlı olduğunu göstermiştir (MD:-3.29; %95:[-4.48, -2.10]; Z=5.41; p<0.0001). Çalışmanın birleştirilmiş sonuçları, günde 3 kez akupresur girişimi sonrasında hastaların konstipasyon düzeyinde 2.59 puan azalma olduğu ve bu azalmanın istatistiksel olarak anlamlı olduğunu göstermiştir (MD:-2.59; %95:[-3.67, - 1.51]; Z=4.69; p<0.0001). Girişim sonrasında hastaların konstipasyon düzeyinde Bristol Dışkı Formu ölçeğinde 0.72 puan artışın olduğu ve bu artmanın istatistiksel olarak anlamlı olduğunu göstermiştir (MD:-0.72; %95:[0.51, 0.94];Z=6.66; p<0.0001). Çalışmanın birleştirilmiş sonuçları girişim sonrasında hastaların yaşam kalitesi ölçeğinde 1.31 puan deney grubunun lehine azalma olduğu ve bu azalmanın istatistiksel olarak anlamlı olduğu saptanmıştır (MD:-1.88; %95:[-2.64, - 1.11]; Z=4.79; p<0.00001). Sonuç olarak; kemoterapi alan kanser hastalarında konstipasyonu önlemek ve yaşam kalitesini arttırmada akupresur pozitif etkiye sahip bir uygulamadır.

The aim of this systematic review and meta-analysis is to evaluate the existing evidence from randomized controlled trials (RCTs) on the effectiveness of acupressure in preventing constipation in cancer patients undergoing chemotherapy. A comprehensive literature search was conducted between November 1, 2023, and April 1, 2024, in PubMed, Medline, Scopus, ScienceDirect, Web of Science, Cochrane Central Register of Controlled Trials, Dergipark, and YÖK TEZ databases. A total of 8 RCTs were included in this meta-analysis. The normal sample size of the studies was 640 (Intervention: 320, Control: 320). The risk of bias in the studies was assessed using the Revised Cochrane Risk-of-Bias Tool for Randomized Trials (ROB 2). The meta- analysis was performed using the Review Manager 5.4.1 software. The steps followed in this meta-analysis were reported according to the PRISMA 2020 checklist. The combined results of the study indicated a significant reduction in constipation levels by 3.29 points after the intervention (MD: -3.29; 95% CI: [-4.48, -2.10]; Z = 5.41; p<0.0001). The combined results also showed a significant reduction of 2.59 points in constipation levels after acupressure intervention three times a day (MD: -2.59; 95% CI: [-3.67, -1.51]; Z = 4.69; p < 0.0001). Furthermore, there was a statistically significant reduction of 0.72 points in the Bristol Stool Form Scale score following the intervention (MD: -0.72; 95% CI: [-0.51, -0.94]; Z = 6.66; p < 0.0001). The combined results revealed a significant decrease of 1.31 points in the quality of life scale favoring the intervention group (MD: -1.88; 95% CI: [-2.64, -1.11]; Z = 4.79; p < 0.00001). In conclusion, acupressure is an effective intervention for preventing constipation and improving quality of life in cancer patients undergoing chemotherapy.
