Hemodiyaliz Uygulanan Hastalarda Hastalık Algısının, Umudun ve Hasta Güçlendirmenin İncelenmesi

Abstract

Amaç: Bu çalışma hemodiyaliz uygulanan hastalarda hastalık algısı, umut ve hasta güçlendirme düzeyleri arasındaki ilişkinin incelenmesi amacıyla yapılmıştır. Gereç ve Yöntem: Tanımlayıcı ve kesitsel tipte olan bu araştırma, İstanbul’da Kasım 2023 ile Ocak 2024 tarihleri arasında 253 hemodiyaliz hastası ile yapıldı. Çalışma verileri, “Kişisel Bilgi Formu, Hastalık Algısı Ölçeği, Sürekli Umut Ölçeği, Hasta Güçlendirme Ölçeği” ile yüz yüze görüşme yoluyla toplandı. Bulgular: Hastaların yaş ortalaması 57,13±14,32 yıl, hemodiyalize girme süresi 71,34±65,6 ay olup Hastalık AlgısıÖlçeği alt boyutlarından Hastalık tipi 4,43±2,99, Süre (akut/kronik) 18,35±1,99, Sonuçlar 19,39±5,16, Kişisel kontrol 18,52±3,03, Tedavi kontrolü 17,12±2,98, Hastalığı anlayabilme 13,79±3,94, Süre (döngüsel) 14,62±3,36, Duygusal temsiller 18,56±6,2, Psikolojik atıflar 18,69±5,2, Risk faktörleri 21,27±6,72, Bağışıklık 9,68±2,77, Kaza veya şans 5,33±2,08 bulundu. Sürekli Umut Ölçeği 39,64±11,32, Hasta Güçlendirme Ölçeği alt boyutlarından Karar alma 3,33±0,8, Kimlik özdeşlik 3,12±0,98, Kişisel kontrol 3,36±1,02, Bilme anlama 3,42±0,96 ve Başkaları ile etkileşim 3,26±1,1 puan bulundu. Sürekli Umut Ölçeği ile sonuçlar (r=-,300, p<0,01), hastalığı anlayabilme (r=-,337, p<0,01) ve duygusal temsiller (r=-,191, p<0,01) arasında ve Hasta Güçlendirme Ölçeği ile sonuçlar (r=-,364, p<0,01), Hastalığı anlayabilme(r=-,448, p<0,01) ve Duygusal temsiller (r=-,279, p<0,01) arasında negatif ilişki saptandı. Sürekli Umut Ölçeği ve alt boyutları ile Hasta Güçlendirme Ölçeği arasında pozitif ilişki saptandı (r=,762, p<0,01). Sonuç: Hastalığın negatif sonuçlarının, kişinin hastalığını iyi kavrama düzeyini ve kişinin hastalığıyla ilgili endişelerini, umut ve güçlendirme düzeylerini azalttığı belirlendi. Hastaların umut düzeyi artıkça güçlendirme düzeylerinin de arttığı saptandı.

Aim: This study aimed to examine disease perception, hope, and patient empowerment in patients undergoing hemodialysis. Materials and Method: A descriptive cross-sectional study was conducted with 253 hemodialysis patients in Istanbul between November 2023 and January 2024. Data were collected through face-to-face interviews using a Personal Information Form, the Illness Perception Scale (IPS), Dispositional Hope Scale (DHS), and Patient Empowerment Scale (PES). Results: The study included 253 hemodialysis patients in Istanbul with a mean age of 57.13±14.32 years. The average hemodialysis duration was 71.34±65.6 months. The subdimensions of the Illness Perception Scale (IPS) yielded the following mean scores: Disease Type 4.43±2.99 points, Duration (acute/chronic) 18.35±1.99 points, Results 19.39±5.16 points, points, Personal control 18.52±3.03 points, Treatment control 17, 12±2,98 points, Understanding the disease 13,79±3,94 points, Duration (cyclic) 14,62±3,36 points, Emotional representations 18,56±6,2 points, Psychological attributions 18,69±5,2 points, Risk factors 21,27±6,72 points, Immunity 9,68±2,77 points, Accident or chance 5,33±2,08 points. The Continuous Hope Scale was 39.64±11.32 points, alternative ways 20.88±5.82 points, activist thinking 18.76±6.11 points from the sub-dimensions of the Continuous Hope Scale. Patient Empowerment Scale 3,29±0,84 points, Patient Empowerment Scale sub-dimensions Decision making 3,33±0,8 points, Identity identification 3,12±0,98 points, Personal control 3,36±1,02 points, Knowing understanding 3,42±0,96 points and Interaction with others 3,26±1,1 points were found. A negative correlation was found between the Continuous Hope Scale and Outcomes (r=-,300, p<0,01), Understanding the Disease (r=-,337, p<0,01) and Emotional Representations (r=-,191, p<0,01) and between the HOS and Outcomes (r=-,364, p<0,01), Understanding the Disease (r=-,448, p<0,01) and Emotional Representations (r=-,279, p<0,01). A positive correlation was found between the Continuous Hope Scale and its subscales and the Patient Empowerment Scale (r=,762, p<0,01). Conclusion: Perceptions of negative disease consequences, limited disease understanding, and disease-related worries were associated with lower hope and empowerment. Conversely, higher hope scores correlated with increased patient empowerment.
