COVİD-19 pandemisinde görev alan hemşirelerin kadın sağlığı, ikincil travma düzeyi ve yaşam kalitesinin incelenmesi

Abstract

Araştırmanın amacı, COVID-19 pandemisi'nde görev alan hemşirelerin kadın sağlığı, ikincil travmatik stres düzeyi ve iş yaşam kalitesinin incelenmesidir. Araştırma, tanımlayıcı, analitik, ilişkisel tasarımında yapılmıştır. Araştırmada (E-20292139-050.01.04-456) etik kurul onayı, TC Sağlık Bakanlığı (E.1244), gönüllü katılan hemşirelerden ve ölçek sahiplerinden izin alınmıştır. Araştırma örnekleminde 265 hemşireye ulaşılmış ve araştırmanın gücü 0.91'dir. Veriler, "kadın sağlığı, ikincil travmatik stres ve iş yaşam kalitesi öçekleri ile online google form ve yüz yüze toplanmıştır. Veriler SPSS v26.0 da analiz edilmiştir. Araştırmada hemşirelerin %42,6'sı 25 yaş, %53,6'sı evli, %80'i lisans mezunu; %47,9'u haftada >48 saat çalışmakta ve %36,6'sı COVID-19 pandemisinde enfekte olmuştur. Hemşirelerin kadın sağlığı ölçeği toplam puanı 57,52±19,82, ikincil travmatik stres ölçeği toplam puanı 48,87±15,22, iş yaşam kalitesi ölçeği toplam puanı 2,32±0,74'dür. Hemşirelerin yaş grupları, medeni durumu, eğitim düzeyi, çalışma şekli, mesleki deneyimi, kadın sağlığı ölçeği bazı alt boyutlarında; hemşirelerin yaş grupları, 13 ay ve üzerinde COVID-19 enfeksiyonlu hastaya bakım vermesi, ikincil travmatik stres ölçek toplam ve bazı alt boyutlarında; hemşirelerin çalışma şekli, iş yaşam kalitesi ölçeği toplam ve tüm alt boyutlarında etkili bulunmuştur (p<0.05). Hemşirelerin "kadın sağlığı ölçeği ve ikincil travma stres ölçeği, iş yaşam kalitesi ölçeği" toplam alt boyut puan ortalamaları birbiri ile pozitif yönde, güçlü ve orta düzeyde ilişkili (r<0.537;p:0.00), ayrıca iş yaşam kalitesi ile ikincil travma stres ölçek toplam puanları negatif yönde, orta düzeyde (r=0.438; p:0.000) ilişkili bulunmuştur. Araştırmada değişkenler lineer regresyon ile incelendiğinde kadın sağlığı, iş yaşam kalitesini negatif yönde ve anlamlı düzeyde yordamakta (p≤0.001) ve "gece ve gündüz" ve "sürekli gece" çalışanların iş yaşam kalitesi sürekli gündüz çalışanlara göre 0.26 kat daha düşüktür. Aile/yakınlarında COVID-19 hastası olan ve kayıp yaşayan hemşirelerin, ikincil travmatik stres düzeyi olmayanlara göre daha yüksektir. Hemşirelerin kadın sağlığı, iş yaşam kalitelerinin iyileştirilmesi, psikososyal açıdan desteklenmeleri, bu alanlarda girişimsel ve nitel çalışmaların yapılması önerilmektedir.

The aim of the study is to examine the women's health, secondary traumatic stress level and quality of work life of nurses who took part in the COVID-19 pandemic. The research was conducted in descriptive, analytical, relational design. Ethics committee approval was obtained for the study (E-20292139-050.01.04-456), permission was obtained from the Turkish Ministry of Health (E.1244), volunteer nurses and scale holders. In the research sample, 265 nurses were reached and the power of the study was 0.91. The data were collected face-to-face and online with the scales of "women's health, secondary traumatic stress and work life quality. Data were analyzed in SPSS v26.0. In the study, 42.6% of the nurses were 25 years old, 53.6% were married, 80% were undergraduates; 47.9% work >48 hours a week and 36.6% have been infected during the COVID-19 pandemic. The nurses' total score on the women's health scale was 57.52±19.82, the total score on the secondary traumatic stress scale was 48.87±15.22, and the total score on the quality of work life scale was 2.32±0.74. Age groups, marital status, education level, working style, professional experience of nurses, some sub-dimensions of the women's health scale; Age groups of nurses, providing care to patients with COVID 19 infection over 13 months, secondary traumatic stress scale total and some sub-dimensions; nurses' working style was found to be effective in total and all sub-dimensions of the work life quality scale (p<0.05). Nurses' total and sub-dimension mean scores of "women's health scale and secondary trauma stress scale, work life quality scale" were positively, strongly and moderately correlated with each other (r<0.537; p:0.00). In addition, quality of work life and secondary trauma stress scale total scores were found to be negatively, moderately correlated (r=0.438; p:0.000). When the variables in the study are examined by linear regression, women's health negatively and significantly predicts the quality of work life (p≤0.001) and the quality of work life of those who work "night and day" and "continuous night" is 0.26 times lower than those who work constantly during the day. Nurses whose family/relatives have COVID-19 patients and who have experienced loss are higher than those who do not have secondary traumatic stress levels. It is recommended that nurses be supported in terms of women's health, work-life quality, psychosocial support, and interventional and qualitative studies in these areas.
