Faaliyet tabanlı maliyetleme analizi: Hemodiyaliz ünitesi üzerine bir uygulama

Abstract

Dünya'da sağlık sistemlerine gün geçtikçe daha fazla mali yük oluşturan hemodiyaliz tedavileri önemli bir finansman sorunu oluşturmaktadır. Diyaliz tedavisi alan hastaların sayısındaki artış ve tedavi maliyetlerinin giderek artıyor olması ülkelerin sağlık bütçelerinden diyaliz tedavileri için yaptıkları harcamaların sürekli olarak artmasına sebep olmaktadır. Bu açıdan bakıldığında hemodiyalizin maliyet analizini yapmak tedavinin finansal boyutunu değerlendirmek amacıyla oldukça önemlidir. Bu çalışma, acil hemodiyaliz tedavi sürecinin maliyetlerini belirleyerek sağlık sistemleri için etkin ve sürdürülebilir stratejilerin geliştirilmesine katkı sağlamayı amaçlamaktadır. Bu araştırmada bir eğitim ve araştırma hastanesi diyaliz ünitesinde yapılan acil hemodiyaliz tedavisine yönelik işlem basamaklarının maliyetleri ortaya konulmaya çalışılmıştır. Bu kapsamda hemodiyaliz ünitesinde kaynak kullanımını optimize etme, bütçe planlamasına katkı sağlama, tedaviye erişimi geliştirme amacıyla faaliyet tabanlı maliyetleme (FTM) ile maliyet analizi yapılması hedeflenmiştir. Araştırma kesitsel ve tanımlayıcı özellikte olup, maliyet analizine konu veriler hastanenin 2022 yılında kayıt altına alınmış mali, idari, tıbbi kayıtlarından hastane bilgi yönetim sistemi üzerinden elde edilmiştir. Tüm bu veriler Microsoft Excel 2021 programında tablolar haline getirilerek analiz edilmiştir. Hesaplamalar neticesinde hemodiyaliz ünitesinde yapılan acil hemodiyalizi hizmeti için seans başına maliyet 1.220,767 TL olarak belirlenmiştir. Çalışma sonucunda elde edilen seans maliyeti 2022 yılı SUT fiyatları ile karşılaştırılmıştır. Acil hemodiyalizinin bir seans maliyeti ile geri ödenen tutar arasında 348,097 TL fark hesaplanmış ve maliyetlerin SUT ödemesinin altında olduğu tespit edilmiştir.

Hemodialysis, which can be counted among the treatment methods that create more and more burden on health systems in the world, constitutes an important health financing problem. The increase in the number of patients receiving dialysis treatment leads to an increase in treatment costs and an increase in health expenditures from the budgets of countries. Cost analysis of hemodialysis is important to evaluate the financial dimension of treatment. This study aims to contribute to the development of effective and sustainable strategies for health systems by determining the costs of the treatment process. In the study, it was tried to reveal the costs of the procedural steps within the scope of emergency hemodialysis treatment performed in a training and research hospital dialysis unit. In this context, it was aimed to conduct a cost analysis study with activity-based costing (ABC) in order to optimize resource use, contribute to budget planning and improve access to treatment in the hemodialysis unit. The research is cross-sectional and descriptive, and the data required for cost analysis were obtained from the hospital's administrative, medical and hospital information management system recorded in 2022. All data were tabulated and analyzed in Microsoft Exel 2021 program. As a result of the calculations, the cost per session for emergency hemodialysis service provided in the hemodialysis unit was determined as 1,220.767 TL. The session cost obtained as a result of the study was compared with the SUT prices of 2022. A difference of 348.097 TL was calculated between the cost of one session of emergency hemodialysis and the reimbursed amount, and it was determined that the costs were below the SUT payment
