Mesane doluluğunun kan basıncı üzerine etkisi

Abstract

Bu çalışma mesane doluluğunun kan basıncı (KB) üzerine etkisi belirlemek amacıyla üç örneklemde gerçekleştirildi. Sağlıklı erişkinlerden oluşan ilk örneklemde (N=63) mesane boş iken, bireyler en az üç saat idrarını tuttuktan sonra sıkışma hissi olduğunda ve mesane boşaltıldıktan hemen sonra; sağlıklı erişkinlerden oluşan ikinci örneklemde (N=80) mesane boş iken, en az 1000 ml sıvı yüklemesi ile mesane doldurulup sıkışma hissi olduğunda ve mesane boşaltıldıktan hemen sonra; ürodinami polikliniğine başvuran hastalardan oluşan üçüncü örneklemde ise (N=92) mesane boş iken, takiben üroflow işlem öncesi mesane doluluğunda ve işlem sonrası mesane boşaltımından hemen sonra KB ölçümleri yapıldı. KB ölçümü oturur pozisyonda, çıplak koldan osilometrik otomatik KB aleti ile yapıldı. Zaman 1 (boş mesane), zaman 2 (dolu mesane) ve zaman 3 (boş mesane) KB ölçüm sonuçları tek yönlü tekrarlı ölçümler ANOVA testi ile karşılaştırıldı. Her üç örneklemde de zaman 2'de sistolik KB sırasıyla 9.2, 12.8, ve 17.4 mmHg (P<0.001); diyastolik KB 5.6, 5,4 ve 7.3 mmHg (P<0.001) artış gösterdi, zaman 3'te ise başlangıç değerlerine indi. Sonuç olarak mevcut çalışma mesane doluluğunun KB'nı yükselttiğini, bu nedenle de KB ölçülmeden önce mesanenin boşaltılması gerektiğini ortaya koymuştur.

This study was carried out with three groups to determine the effect of bladder fullness on blood pressure (BP). In the first group of healthy adults (N=63), BP measurements were made when the bladder was empty and individuals felt urgency after holding their urine for at least three hours, and immediately after the bladder was emptied. In the second group of healthy adults (N=80), BP measurements were made when the bladder was empty, after the bladder was filled with at least 1000 ml of fluid loading and individuals felt urgency, and immediately after the bladder was emptied. In the third group (N=92), which consisted of patients presenting to the urodynamics outpatient clinic, BP measurements were made when the bladder was empty, followed by when the bladder was full before the uroflow procedure and immediately after bladder emptying after the procedure. BP was measured in the sitting position, using an oscillometric automatic BP instrument from the bare arm. Timepoint 1 (empty bladder), timepoint 2 (full bladder) and timepoint 3 (empty bladder) BP measurement results were compared with one-way repeated measurements ANOVA test. In the three groups, systolic BP at timepoint 2 increased by 9.2, 12.8, and 17.4 mmHg, respectively (P<0.001) and diastolic BP increased by 5.6, 5.4, and 7.3 mmHg (P<0.001) and decreased to baseline values at timepoint 3. In conclusion, the present study demonstrated that bladder fullness increases BP, so the bladder should be emptied before BP can be measured.
