Kemoterapi uygulanan meme kanserli hastalarda hasta aktivasyonu ve semptom yönetiminin değerlendirilmesi

Abstract

Bu çalışma kemoterapi uygulanan meme kanserli hastalarda hasta aktivasyonu ve semptom yönetimini değerlendirmek amacıyla yapılmıştır. Çalışma, İstanbul'da bulunan bir kamu hastanesinin kemoterapi ünitesinde meme kanseri tanısı konmuş ve kemoterapi uygulanan 122 hasta ile Kasım 2020-Haziran 2021 tarihleri arasında yürütülmüştür. Araştırma verileri, Hasta Bilgi Formu, Meme Kanserinde Kemoterapi Semptom Yönetimi-Öz Etkililik Ölçeği ve Hasta Aktivasyonu Ölçeği (PAM) ile yüz yüze görüşme yoluyla ile toplanmıştır. MKKSYÖEÖ puan ortalaması 134.89±44.76 dır. MKKSYÖEÖ alt boyutlarından Problem Çözme puan ortalaması en yüksek iken Kemoterapi Semptom Yönetimi puan ortalaması en düşük olarak bulunmuştur. PAM ham puan değeri ortalaması 36.88±6.07 olup dönüştürülmüş puan ortalaması 52.19±14.31'dir. Eğitim durumuna göre MKKSYÖEÖ ve PAM değeri; sağlık durumu, günlük yaşam aktivitelerini yaparken destek alma, hastalıkla ilgili eğitim alma ve eğitimi yeterli bulma durumu göre MKKSYÖEÖ ve PAM değeri arasında istatistiksel olarak anlamlı farklılık göstermektedir. Yaş ile Problem Çözme ve PAM puanı arasında negatif yönde anlamlı bir ilişki bulunmuştur. PAM değişkeninin MKKSYÖEÖ üzerinde pozitif ve anlamlı bir etkiye sahip olduğu görülmüştür. Sonuç olarak, hastaların semptom yönetimi orta düzeyde, hasta aktivasyonu seviye 2'dedir. Hastaların aktivasyon seviyeleri arttıkça semptom yönetimi artmaktadır. Düşük eğitim düzeyi, kötü sağlık durumu, hastalıkla ilgili eğitim almama ve eğitimi yeterli bulmama semptom yönetimini ve aktivasyonu olumsuz etkilemektedir.

This study was conducted to evaluate patient activation and symptom management in breast cancer patients undergoing chemotherapy. The study was conducted between November 2020 and June 2021 with 122 patients diagnosed with breast cancer and treated with chemotherapy in the chemotherapy unit of a public hospital in Istanbul. Research data were collected through face-to-face interviews with the Patient Information Form, the Chemotherapy Symptom Management in Breast Cancer-Self-Efficacy Scale (CSMBCSES) and the Patient Activation Scale (PAS). The mean score of CSMBCSES is 134.89±44.76. While the average of Problem Solving scores from the sub-dimensions of CSMBCSES was the highest, the mean of Chemotherapy Symptom Management was found to be the lowest. The mean PAM raw score was 36.88±6.07, and the mean converted score was 52.19±14.31. CSMBCSES and PAS value according to education level; There is a statistically significant difference between CSMBCSES and PAS values according to health status, getting support while doing daily living activities, getting education about the disease and finding the education sufficient. A significant negative correlation was found between age, Problem Solving and PAS score. It was observed that the PAS variable had a positive and significant effect on CSMBCSES. As a result, patients' symptom management is moderate and patient activation is level 2. As the activation levels of the patients increase, symptom management increases. Low education level, poor health status, lack of education about the disease and insufficient education affect symptom management and activation negatively.
