Üremeye yardımcı tedavi gören bireylerin evlilik uyumu ve manevi iyi oluşlarının incelenmesi

Abstract

Kadın ve erkeklerin bir araya gelerek bir "Aile" oluşturması ve aile olarak kalabilmesi her zaman olanaklı görünmemektedir. Uygun şartların bir araya gelişiyle gerçekleşen her evlilik tanımı, çocuk durumuna bağlı olarak aile kavramı ile ifade edilememektedir. Evlilik ve eşler arası uyumun temel bir dengeyi barındıran doğası, çocuk sahibi olmak isteyen bireylerin doğal yollarla karşılayamadıkları bir süreç ile sarsılabilmektedir. Korunmasız 1 yıllık cinsel ilişki sonrasında sağlıklı bir gebeliğin gerçekleşmemiş olması, üremeye yardımcı tedavilere (ÜYT) başvurmak için yeterli bir durumdur Bu çalışmada üremeye yardımcı tedavi gören kadınların süreç içerisindeki tecrübeleri, yaşadıkları zorluklar, psikolojik duygu durumları, evlilik uyum ve manevi iyi oluş durumları bütünsel bir olgu olarak incelenmiştir. Betimsel yorumlayıcı olgubilime (fenomonoloji) dayalı olan bu durum çalışmasına Kocaeli ve Sakarya illerinde amaçlı örnekleme yöntemlerinden kritik durum örneklemesi ve kartopu örnekleme teknikleri ile belirlenen infertilite tedavisi almış 20 kadın katılmıştır. Çalışmanın veri kaynağı katılımcılar ile yüz yüze derinlemesine yapılan yarı- yapılandırılmış görüşmelerdir. Bu olgunun onu yaşayan kadınlar tarafından nasıl algılandığı ve anlamlandırıldığının anlaşılması veri analizi sürecinin odağını oluşturmuştur. Derinlemesine yapılan görüşmeler neticesinde ortaya çıkan bulgular ÜYT sürecine, evlilik uyumuna, maneviyatın başa çıkmaktaki etkisine göre kodlanmış olup bu ana başlıklar altında değerlendirilmiştir. Temel olarak katılımcılar ÜYT süreçlerinde karşılaştıkları zorluklar neticesinde evlilik uyumları, kriz anında kenetlenme ve diğerkâm bakış ile olumlu yönde etkilenmiş görünmektedir. Evlilik uyumlarını olumsuz etkileyen başlıklarsa, aile ve toplumdan uzaklaşma, etiketlemeler, maddiyat, eş ve ailesinden gelen sorular ve boşanma teklifleri şeklinde ortaya çıkmaktadır. Çalışmada manevi danışmaya ilişkin ortaya çıkan başlıca temalar; imtihan olarak görme, tevekkül ve teslimiyet, hayra yorma, manevi-ibadi terkipler, mucize olgusu, manevi büyükler, psişik deneyimler gibi oldukça zengin bir yapıdadır. Katılımcılar tedavi süreçlerini anlamlandırmakta, zorluklarla başa çıkmakta ve sürecin şartlarına uyum sağlamak için zengin manevi envanterden destek almaktadırlar. Çalışmanın sonuçları evlilik kurumu ve çocuk sahibi olmakta güçlük çeken bireylere yönelik toplumsal algı ile ilgili yeni bir bakış açısı sunmakta ve bununla birlikte tedavi hizmeti veren kurumlara kendi hastalarının mevcut psikolojik ve manevi iyi oluş durumları hakkında bilgi vermektedir.

It does not always seem possible for men and women to come together to form a "Family" and stay as a family. Depending on the child status not all marriages can be defined with the word "family". With the process of desire to having a child and not being able to achieve this goal in natural way the sensitive natural balance of the marriage and the marital adjustment can be shaken. Failure to achieve pregnancy after one year regular unprotected sexual intercourse is a valid reason for attaining artificial reproductive treatment. The participants of this case study , which is based on descriptive interpretative phenomenology, were 20 women who received the treatment. Participants were determined by the critical case and snowball sampling techniques from the purposeful sampling methods in Kocaeli and Sakarya provinces. The qualitative data were collected with face to face in-depth semi-structured interviews with participants. Understanding how this phenomenon is perceived and made sense by women living it has been the focus of the data analysis process. The findings that emerged as a result of the in-depth interviews were coded according to the treatment process, marital adjustment, and the effect of spirituality in dealing with these situation. Findings are gathered under the following titles: treatment process, marital adjustment and role of the morality in coping. Basically, the participants seem to have been positively affected by their marital adjustment as a result of the difficulties they faced in the treatment processes, on the ground of altruism, with interlocking at the time of crisis. Topics which have a negative effects on marital adjustment have been emerged as alienation from family and society, labeling, financial difficulties, questions from spouse and his/her family and divorce proposals. The main themes that emerged regarding spiritual counseling varied from considering the situation as a divine test, trusting and surrendering to the god, benevolence, spiritual-worship compositions, miracle phenomenon to spiritual elders, and psychic experiences. Participants are triying to make sense of their treatment processes, cope with difficulties and receive support from the rich spiritual inventory to adapt to the conditions of the process. The results of the study provide a new perspective on the social perception of individuals who facing difficulties both in their marriage and in having children, and also provide information to the institutions which providing treatment services about the current psychological and moral well-being of their patients.
