Postpartum depresyona yönelik sosyal hizmet uygulamaları: Vaka sunumu

Abstract

Postpartum depresyon, annelerde gebelik öncesinde başlayabilen ve doğum sonrasında uzun bir süreci içerebilen üzüntü, hüzün, ilgi kaybı, yorgunluk, uyku ve iştahta düzensizlik, içe çekilme ve dikkati toplayamama gibi depresif duygudurum halidir. Postpartum depresyonda annenin bebeğe yönelik ilgi eksikliği, bebekle bağ kurmada zorlanma, iyi bir anne olmadığı için kendini suçlama, bebeğe bakma yeteneğinden şüphe etme, bebeği istemiyormuş gibi hissetme, kendine, bebeğe veya partnere zarar verme endişesi duyma gibi durumlar sıklıkla görülmektedir. Postpartum depresyonun ebeveyn (anne, baba ve çocuk ilişkisi), eş ve varsa kardeş alt sistemleri üzerinde biyopsikososyal yönden pek çok olumsuz etkisi bulunmaktadır. Sosyal hizmet uzmanları çocuğun yüksek yararı ilkesinden hareketle postpartum depresyonun olumsuz etkilerinin önlenmesine ve ortadan kaldırılmasına yönelik mesleki müdahaleleri gerçekleştirmektedir. Bu çalışma kapsamında postpartum depresyon çerçevesinde kurgusal bir vakaya yönelik planlı değişim sürecini içeren sosyal hizmet müdahalesi örneği paylaşılmaktadır. Hastanenin tıbbi sosyal hizmet birimine hekim tarafından konsülte edilen vakada postpartum depresyon yaşayan anne, baba ve bebek arasındaki sorunların çözümüne odaklanılmıştır. Ailenin yaşadığı sorunlar çevresi içinde birey anlayışı ile analiz edilip, sosyal destek mekanizmaları harekete geçirilerek vaka formülasyonunun yapılması amaçlanmıştır.

Postpartum depression is a depressive mood state such as sadness, sadness, loss of interest, fatigue, sleep and appetite disorder, withdrawal and inability to concentrate, which can start before pregnancy and can include a long period after birth. In postpartum depression, the mother's lack of interest in the baby, difficulty in bonding with the baby, self-blame for not being a good mother, doubting her ability to take care of the baby, feeling like she doesn't want the baby, and worrying about harming herself, the baby or the partner are common. Postpartum depression has many biopsychosocial negative effects on parent (mother, father and child relationship), spouse and sibling subsystems, if any. Social workers perform professional interventions to prevent and eliminate the negative effects of postpartum depression, based on the principle of the best interests of the child. Within the scope of this study, an example of social work intervention that includes a planned change process for a fictional case within the framework of postpartum depression is shared. In the case, which was consulted by the physician to the medical social work unit of the hospital, the focus was on solving the problems between the mother, father and baby who had postpartum depression. It is aimed to analyze the problems experienced by the family with the understanding of the individual in its environment, and to make case formulation by activating social support mechanisms.
