Down Sendromunda Tıbbi Beslenme Tedavisi

Abstract

Down Sendromu kromozom çiftinin mayoz bölünmesi sırasında 21. kromozomun iki tane olması gerekirken üç tane olması durumu ile gerçekleşen bir anomalidir. Her yaştan ve ırktan insanı etkileyebilmekte olup en çok görülen kromozom anomalisi türüdür. Ekstra kromozom, bireylerde hafıza ve konuşma problemleri, yavaş motor gelişimi ve zekâ geriliği gibi gelişim problemlerine yol açmaktadır. Bireylerde kardiyovasküler sistem, gastrointestinal sistem ve kas-iskelet sistemi hastalıklarının yanı sıra nörolojik düzeyi de etkileyen yandaş hastalıklar mevcut olabilmektedir. Down sendromlu bireylerdeki fiziksel ve motor gelişim geriliği, beslenme güçlüklerine yol açabilmektedir. Sağlıklı bireylere kıyasla Down sendromlu bireylerde obezite görülme sıklığı yüksek bulunmuştur. Obezite prevalansının yüksek olması, bazal metabolizma hızının düşük oluşu, sistemik inflamasyona yatkınlık, azalmış kas tonusu, yüksek enerjili-düşük besin değerli besin tüketimi, hipotiroidizm varlığı ve azalmış fiziksel aktivite gibi hem çevresel hem de genetik faktörlerin etkisi ile ilişkilendirilmiştir. Literatür çalışmaları Down sendromlu bireylerin obezite tedavisinde sağlıklı besin seçimi ve ebeveynlere beslenme eğitiminin gerekliliği ile fiziksel aktivite teşvikinin önemini vurgulamaktadır. Bu derlemede Down sendromlu bireylerin beslenme durumunun araştırılması ve tıbbi beslenme tedavisinin önemine değinilmesi amaçlanmıştır.

Down Syndrome is an anomaly that occurs when the chromosome 21 pair has three chromosomes instead of two during meiosis. It can affect people of all ages and races and is the most common type of chromosomal anomaly. The extra chromosome causes developmental problems in individuals such as memory and speech problems, slow motor development and mental retardation. Individuals may have concomitant diseases that affect the neurological level, as well as cardiovascular system, gastrointestinal system and musculoskeletal system diseases. Physical and motor developmental delay in individuals with Down syndrome can lead to feeding difficulties. The prevalence of obesity was found to be higher in individuals with Down syndrome compared to healthy individuals. The high prevalence of obesity has been associated with the effects of both environmental and genetic factors, such as low basal metabolic rate, susceptibility to systemic inflammation, decreased muscle tone, consumption of high-energy-low-nutrient food, presence of hypothyroidism and decreased physical activity. Literature studies emphasize the importance of healthy food selection, the necessity of nutritional education for parents and the promotion of physical activity in the obesity treatment of individuals with Down syndrome. In this review, it is aimed to investigate the nutritional status of individuals with Down syndrome and to emphasize the importance of medical nutrition therapy.
