Yoğun bakım ünitelerinde çalışan hemşirelerin tamamlayıcı ve alternatif tedavi yöntemlerine ilişkin uygulama ve tutumlarının belirlenmesi

Abstract

Yoğun Bakım Ünitelerinde görev alan hemşirelerin Tamamlayıcı ve Alternatif Tedavi (TAT) yöntemlerine ilişkin uygulama ve tutumlarını belirlemek için yapılan bu araştırma tanımlayıcı ve kesitsel tiptedir. Araştırmanın evrenini İstanbul Başakşehir Çam ve Sakura Şehir Hastanesinde yoğun bakım bölümünde çalışan 252 hemşire oluşturmaktadır. Veriler, Hemşire Tanıtıcı Bilgi Formu ile Bütünleyici Tamamlayıcı ve Alternatif Tedavi Tutum Ölçeği kullanılarak toplandı. Hemşirelerin yaş ortalaması 25,48±1,99 olup, %79,8'i kadın, %79,4''ü bekar ve katılımcıların %90,9'u 2017 yılından önce mezun olmuştur. Hemşirelerin %58,3'ünün 2 yıl ve altı mesleki tecrübesi olduğu belirlendi. Hemşirelerden %60,3'ü TAT teriminin ne anlama geldiğini bildiklerini, %77,4'ü TAT hakkında eğitim almadığını, alanların ise %73,7'si lisans eğitimi sırasında aldıklarını belirtmiştir. Hemşirelerin %77'si kendisi ve ailesi için TAT yöntemlerini kullandıklarını belirtirken en çok bildikleri yöntem olarak %66,7 ile masaj, en çok kullandıkları yöntem olarak %45,2 ile masaj ve en çok uygulamak istedikleri yöntem olarak da %42,9'la masajı tercih etmişlerdir. Hemşirelerin BTATTÖ'den alınan toplam puan ortalaması 25,53±5,97, Tamamlayıcı ve Alternatif Tıp alt boyutu için 16,71±4,13, Bütüncül Sağlık alt boyutu için ise 8,82±3,441'dir ve ölçeğin alfa değeri 0,66 olarak bulunmuştur. TAT güven skalasından alınan toplam puan ortalaması ise 6,40±1,76'dir. Yoğun bakım hemşirelerinden kadınların, 27 yaş ve üstünün, evlilerin, mezuniyetin 2010-2016 yılları arasında olanların, 5 yıl ve üstü hemşire olarak çalışanların ve kronik hastalığı olanların TAT'a ilişkin tutumlarının olumlu olduğu bulundu. Hemşirelerden evli ve yüksek lisans mezunu olanların TAT'a duyulan güven düzeyinde tutumlarının olumlu olduğu bulundu. Araştırmada ortaya çıkan sonuçlar göz önünde bulundurulduğunda, ülkemizde yoğun bakım hemşirelerin TAT yöntemlerinde uygulama ve tutumlarını belirlemeye yönelik daha fazla araştırma yapılması ve TAT'la ilgili eğitimler hazırlanarak, yoğun bakım hemşirelerinin bilgi sahibi olmasının sağlanması önerilmektedir.

This research, conducted to determine the practices and attitudes of nurses working in Intensive Care Units regarding Complementary and Alternative Treatment (CAM) methods, is of a descriptive and cross-sectional type. The universe of the research is composed of nurses working in the intensive care department at Istanbul Başakşehir Çam and Sakura City Hospital. The effect size of the study was determined as 0.05 alpha value for the F test, 0.25 effect size and theoretical power were taken as 95%, and the minimum total number of samples was determined as 252, and research was conducted with 252 nurses. The data were collected using the Nurse's Introductory Information Form and the Integrative Complementary and Alternative Treatment Attitude Scale. Of the nurses with an average age of 25.48±1.99, 79.8% are female, 79.4% are single, and 90.9% graduated in 2017 or older. It was found that 58.3% of nurses had professional experience of 2 years and below. 60.3% of the nurses stated that they knew what the term CAM means, 77.4% of them did not receive training on CAM, and 73.7% of those who received it stated that they received it during their undergraduate education. 77% of nurses stated that they used CAM methods for themselves and their family, while 66.7% preferred massage as the method they know the most, 45.2% preferred massage as the method they use the most, and 42.9% preferred massage as the method they want to apply the most. The mean total score of the nurses obtained from the BTATTO was 25.53±5.97, 16.71±4.13 for the Complementary and Alternative Medicine sub-dimension, 8.82±3.441 for the Holistic Health sub-dimension, and the alpha value of the scale was found to be 0.666. The mean total score obtained from the CAM confidence is 6.40±1.76. It was found that among the intensive care nurses, women, 27 years and older, married, graduated between 2010-2016, working as a nurse for 5 years or more, and those with chronic diseases had positive attitudes towards CAM. It was found that the attitudes of those who are married to nurses and have a master's degree at the level of trust in TAT were positive. In line with the results obtained from the study, it was suggested that more research should be conducted to determine the practices and attitudes of intensive care nurses in CAM methods in our country, and trainings on CAM should be organized to ensure that nurses have knowledge on this subject.
