Bariatrik cerrahi öncesi uygulanan diyetin dna metilasyonu aracılığıyla ghrelin düzeyine etkisi

Abstract

Çalışmamızın amacı bariatrik cerrahi öncesi uygulanan diyetin Ghrelin geninde meydana gelen DNA metilasyonu ve ağırlık kaybı üzerine etkisini araştırmaktır. Çalışmamızın evrenini Sağlık Bilimleri Üniversitesi Bakırköy Dr. Sadi Konuk Eğitim ve Araştırma Hastanesi Genel Cerrahi Ana Bilim Dalı'na başvuran ve bariatrik cerrahi operasyonu geçiren 50 morbid obez hasta oluşturmaktadır. Hastalar randomize kontrollü olarak iki gruba ayrılmıştır. Çalışma grubu pre-op iki hafta süreyle çok düşük kalorili protein ağırlıklı bir beslenme programı uygularken kontrol grubu pre-op dönemde herhangi bir beslenme programı uygulamamıştır. Değişkenlerin istatistiksel olarak değerlendirmesi Student t test, Mann Whitney U test, paired sample t testi, Wilcoxon işaret testi, Fisher's Exact Ki-Kare testi, Fisher Freeman Halton Exact Ki-kare testi, Continuity (Yates) Düzeltmesi ve Mc Nemar testi kullanılarak gerçekleştirilmiştir. Araştırmamızın sonucunda diyet uygulayan grubun post-op süreçte aşırı ağırlık kaybı yüzdelerinin diğer gruba göre anlamlı derecede yüksek olduğu sonucuna varılmıştır (p<0.05). Ağırlık kaybına bağlı olarak karaciğer yağlanmaları da çalışma grubunda anlamlı derecede azalma göstermiştir (p<0.05). Ghrelin geni DNA metilasyonu ise çalışma grubunda anlamlı derecede daha yüksek bulunmuştur (p<0.05).

The aim of our study is to investigate the effect of diet applied before bariatric surgery on DNA methylation in the Ghrelin gene and weight gain. Our study consists of 50 morbidly obese patients who underwent bariatric surgery, who applied to the Department of General Surgery, Bakırköy Dr Sadi Konuk Training and Research Hospital, University of Health Sciences. The patients were divided into two groups as randomized controlled. While the study group applied a very low calorie protein diet for two weeks before the surgery, the control group did not apply any nutrition program in the pre-op period. Statistical evaluation of the variables was performed using Student t test, Mann Whitney U test, paired sample t test, Wilcoxon sign test, Fisher's Exact Chi-square test, Fisher Freeman Halton Exact Chi-square test, Continuity (Yates) Correction and Mc Nemar test. As a result of our research, it was concluded that the percentage of excessive weight loss in the post-op period of the study group was significantly higher than the other group (p<0.05). In the same group depending on weight loss, fatty liver also decreased significantly (p<0.05). Ghrelin gene DNA methylation was found to be significantly higher in the study group (p<0.05).
