Sağlık turizmi tahsilat oranlarının incelenmesi: Bir kamu hastanesi örneği

Abstract

Bu çalışmanın temel amacı, sağlık turizmi kapsamında verilen sağlık hizmeti ücretlerinin tahakkuk ve tahsilat oranlarını ile tahakkuk eden ücretlerin tahsil edilememesinin sebeplerini ortaya koymaktır. Tez araştırmasında iki türlü veri kullanılmıştır. Birinci veri türü, hastaların demografik verileri, klinik verileri ve ödeme verilerini kapsayan ikincil veri içeren bir veri setinden oluşmaktadır. İstanbul ilinde bulunan A1 kategorisindeki kamu hastanesinin Hastane bilgi yönetim sisteminden çekilecek olan ikincil veriler için bir evren veya örneklem seçimine gidilmemiş, Sadece 2021 Ocak ile 2021 Aralık ayı arasındaki sağlık turizmi ve turistin sağlığı verileri kullanılmıştır. Nitel çalışma için ise evren, tez çalışmasının yapılacağı hastanenin tahsil ve tahakkuk oranları ile ilişkisi ve bilgisi olan birimleri kapsamaktadır. Sağlık turistlerinin tahakkuk ve tahsilat oranlarına ilişkin bilgilere bakıldığında gerek sağlık turistlerinin gerek ise de turistin sağlığı kapsamında alınan sağlık hizmetlerinin tahakkuk oranlarının tahsilat oranlarından çok daha yüksek olduğu görülmüştür. Öyle ki sağlık turizmi kapsamındaki hizmet bedellerinin tahakkuk edilen kısmı sağlık turizmi kapsamındaki hizmet bedelinin tahsil edilen ortalamasından çok daha yüksektir. Aynı şekilde turistin sağlığı kapsamındaki tahakkuk ve tahsilat oranları da birbirinden farklılık göstermektedir. Ancak turistin sağlığı kapsamındaki tahakkuk ve tahsilat oranları arasındaki fark çok daha fazladır. Başvuran hastaların çoğunluğunu sosyoekonomik düzeyi düşük ülke vatandaşlarının oluşturduğunu, sağlık turizmin-den çok turistin sağlığı başvurusu olduğu görülmüştür. Başvurulan branşlara bakıldığında turistin sağlığı hastaları genelde acil durumlar için hastaneye başvurduğu görülmüştür.

The main purpose of this study is to reveal the accrual and collection rates of health service fees given within the scope of health tourism and the reasons for not collecting the accrued fees. Two types of data were used in the thesis research. The first data type consists of a dataset with secondary data that includes patient demographics, clinical data, and payment data. A population or sample was not selected for the secondary data to be drawn from the Hospital information management system of the A1 category public hospital in Istanbul, only the health tourism and tourist health data between January 2021 and December 2021 were used. For the qualitative study, the universe includes the units that have knowledge and relationship with the collection and accrual rates of the hospital where the thesis study will be conducted. When the information on the accrual and collection rates of health tourists is examined, it is seen that the accrual rates of health services received within the scope of health tourists and tourists are much higher than the collection rates. In fact, the accrued portion of the service fees within the scope of health tourism is much higher than the average collected service fee within the scope of health tourism. Likewise, the accrual and collection rates within the scope of tourist health also differ from each other. However, the difference between the accrual and collection rates within the scope of tourist health is much larger. It has been observed that the majority of the applicants are citizens of countries with low socioeconomic status, and they are the applications for the health of tourists rather than health tourism. Considering the branches applied, it was seen that the tourist's health patients generally applied to the hospital for emergencies.
